- We are hiring throughout Albany / Schenectady / Rensselaer Counties!
- This is a Hybrid position, some field work is required.
- Offering $26.00 Hourly

Schedule and coordinate the assessment process with individuals, families and providers in the home and/or community. 2. Must be willing to travel throughout the assigned region (when time permits) 3. Contact the individual and/or family/caregiver to schedule the appointment at a time and location most convenient to that person. This will include nights and weekends at the person's request. 4. Arrange formal interpreter services as needed. 5. Identify if the individual is capable of participating in an interview (full or partial) and if there is a need for an observation. More than one visit may be required. 6. Conduct the interview/observation in person. Assessment cannot be complete without the person being interviewed or observed. 7. Utilize a person-centered interview process (e.g. conversational style that starts with the person).Required Experience/Licensure:
Minimum of one year post-educational experience working with adult individuals with intellectual and/or developmental disabilities Valid NYS Driver's license or State ID must be presented on first day of training A completed Bachelor's degree in a human services field Must be open to schedule including day/evening and weekend shifts Must meet in-field geographic coverage expectationsRequired Skills:
Strong analytical skills; detail and solution oriented; ability to work independently; excellent written and oral communication and presentation skills; and ability to use spreadsheet or database software; Proficient computer skills including, but not limited to, the ability use the following on a daily basis: Web-based applications, Email, Microsoft Word.
