(FHLC) is seeking a compassionate, organized, and highly motivated SSVF Case Manager to help Veterans and their families achieve and maintain housing stability throughout Palm Beach County. Our Case Managers serve as the primary point of contact for participating Veteran households. This role combines direct case management, housing stabilization planning, resource coordination, financial assistance, advocacy, and detailed documentation. The ideal candidate is an excellent communicator who can build strong relationships with participants while remaining organized, accountable, tech-savvy, and attentive to program requirements. What You'll Do Complete participant assessments and develop individualized Housing Stability Plans. Maintain consistent contact with Veteran households based on participant needs and program requirements. Help participants identify barriers, establish goals, and work toward long-term housing stability. Coordinate supportive services and referrals, including healthcare, benefits, employment, legal assistance, public benefits, and other community resources. Initiate Temporary Financial Assistance (TFA) requests when allowable and supported by participant need and documentation. Communicate and coordinate with landlords and property managers when authorized and appropriate. Collaborate with Housing Navigators, Healthcare Navigators, VA partners, Legal Aid, CareerSource, and other community providers. Complete certifications, recertifications, service entries, Housing Stability Plans, budgets, referrals, and case closure activities. Maintain complete and compliant participant files in accordance with SSVF grant requirements. Documentation Matters Strong documentation isn't an afterthought in this position—it is an essential part of providing quality case management. If it isn't documented, it didn't happen.
The ideal candidate will have: Excellent verbal, written, and interpersonal communication skills. Strong organization, time-management, and follow-through. Excellent attention to detail and documentation. Strong technology skills and the ability to learn new electronic systems quickly. Comfort using Microsoft Office, Outlook, Teams, electronic databases, and document management systems. Ability to manage a caseload while balancing multiple priorities and deadlines. Strong critical-thinking and problem-solving skills. Ability to establish appropriate professional boundaries while building positive participant relationships. Ability to work independently while collaborating with a multidisciplinary team. Compassion and professionalism when working with individuals experiencing homelessness, housing instability, trauma, or other complex barriers. Preferred Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, Public Health, or a related field. Relevant professional experience may be considered in place of a degree. Previous experience in case management, housing services, homeless services, Veteran services, behavioral health, or another human-services setting. Familiarity with

Case Managers are expected to:
Complete accurate and timely HMIS case notes and service entries. Document participant contacts, contact attempts, referrals, barriers, progress, and services provided. Maintain assessments, Housing Stability Plans, budgets, TFA documentation, certifications, recertifications, and closure documents. Upload and organize required documentation within our electronic document management system. Ensure participant files are complete, current, and compliant with SSVF grant requirements. Respond promptly to file audits and requests for documentation corrections. resources, housing programs, or Continuum of Care systems is preferred. Schedule & Work Environment This is a full-time position, Monday through Friday from 8:00 AM-5:00 PM. All initial training is completed in person at our office during regular business hours. New Case Managers should expect to report to the office daily throughout their training period. Following successful completion of training, Case Managers are required to work in the office a minimum of one day per week , with additional in-office or community-based work required based on participant, program, meeting, training, or supervisory needs.Occasional schedule adjustments or additional hours may be necessary to meet participant or program needs.
