Role:
The Community Choice Guide II helps aging and disabled adults maintain or transition into stable, community-based housing. This is a field-based role involving frequent visits to client homes, care settings, and community locations across Clark and/or Cowlitz Counties. You'll support clients with housing, resources, paperwork, and stabilization needs while collaborating closely with case managers.
Core Responsibilities:
Contact new clients within 48 hours of referral Meet weekly with clients and support goal-setting Assist with housing searches, applications, vouchers, and move-in tasks Provide stabilization support (e.g., hoarding intervention, caregiver searches) Ensure medical and caregiving resources are in place Maintain accurate documentation (case notes, MARs, time logs) Submit purchase orders with approval and track item delivery Provide weekly updates to case managers Work collaboratively with case managers and behavioral support staff
Work Environment:
This is a field-based position . You must be comfortable driving frequently and entering a variety of living environments. Some clients may be in unstable or temporary housing situations, so flexibility, empathy, and professionalism are essential to be successful.
Disclaimer:
A major perk of this position is the flexibility to create your own schedule after the initial training period. You'll have the freedom to work from home, in the field visiting clients, and from the office two days per week which will be arranged prior to hire. After training, there are limited mandatory office days, about 8 hours per week. However, during the first 90 days , the schedule is structured and requires availability in the office in Vancouver, WA and in the field from 8:45 AM to 5:15 PM, Monday through Friday . This ensures proper onboarding, training, and support as you get started in your role.
