Job Description

TriSource is recruiting for a nonprofit organization dedicated to preventing homelessness and providing housing solutions for individuals and families across Southeast Michigan. Since 2001, They have served as a leading housing resource organization, supporting people experiencing homelessness, individuals with disabilities, low-income households, and other vulnerable populations. We are seeking a Housing Case Manager to provide housing-focused case management services and support participants in achieving long-term housing stability. This role requires a compassionate, strengths-based approach and a commitment to serving vulnerable populations. This is a temporary position until September 30th, 2026 Work Schedule & Environment This is a hybrid position consisting of:

Remote work and community-based client visits Regular home visits with participants are required Candidates must be comfortable conducting home visits and meeting clients in community settings Position Summary The Housing Case Manager is responsible for providing strength-based housing case management services while engaging participants in supportive, solution-focused conversations that promote housing stability. You will work closely with participants, community partners, and service providers to connect individuals and families with housing resources and supportive services. Essential Responsibilities Meet with participants to complete required paperwork and conduct assessments for housing programs and available resources Provide housing case management utilizing Housing First and Critical Time Intervention (CTI) models Develop individualized housing stability plans in collaboration with participants Assist participants in identifying and securing long-term affordable housing opportunities Conduct monthly home visits to review housing goals, eligibility, and transition planning Maintain accurate and timely documentation within the Homeless Management Information System (HMIS) Connect participants to community resources and support services based on household needs Provide crisis intervention and general participant support as needed Participate in coordinated entry activities and collaborate with community partners Stay current on best practices, housing resources, and program requirements through ongoing training and professional development Qualifications Bachelor's degree in Social Work, Human Services, or a related field preferred 2 years of case management experience preferred Experience working with vulnerable populations, including individuals experiencing homelessness, domestic violence survivors, individuals with disabilities, or those at risk of homelessness Strong verbal and written communication skills Ability to work independently and collaboratively in a fast-paced environment Knowledge of housing resources, coordinated entry systems, and community-based services preferred Strong organizational, documentation, and computer skills Detail-oriented with the ability to manage multiple priorities effectively Requirements Valid driver's license Reliable personal vehicle for travel throughout the communities served (gas mileage provided) Ability to conduct regular home visits and community-based meetings with participants
