This is a Contract opportunity.

Remote - Madison/Bond, IL | Jackson, IL | Peoria, IL | Ogle, IL | Cook North, IL (60302) | Lake County, IL ; 25-40% local travel required

Hours/Schedule:

Monday-Friday, 40 hours; 8:00 AM-5:00

W2

Core Requirements:

At least 2 years of healthcare experience, preferably in care management or medical/behavioral health, or an equivalent combination of relevant education and experience. Knowledge of community resources and ability to support member assessments and care coordination. Strong communication, organization, prioritization, critical thinking and Microsoft Office skills; ability to work independently with diverse populations. Valid unrestricted driver's license, reliable transportation and adequate auto insurance for local travel, unless otherwise required by law.

We Consider It A Bonus If You Also Have:

Care management experience. Experience in medical and/or behavioral health settings.

Job Responsibilities:

Complete member assessments and identify care coordination needs. Develop and monitor care plans with members, caregivers and healthcare professionals. Manage member outreach and conduct telephone, in-person or home visits as required. Coordinate community, behavioral health and long-term support services. Facilitate interdisciplinary collaboration, document progress and help address barriers to care.
