Case Manager II - Waiver Case Management (St. Louis County)

Duluth, Minnesota Case Manager II We are seeking a Case Manager II to work on our Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. This is a full-time hybrid position in St. Louis County. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. This will be working with clients in the north and south areas within St. Louis County. We offer work-life balance with reasonable case loads of an average of 30!

Must have physical presence in St. Louis County. Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services. Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received. Assessing client needs and ensuring continuity of care. Making ongoing referrals to resources based on assessment and current client need. Maintaining necessary daily records for continuity of service and reports to funding agencies. Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards. Attending client monthly meetings to monitor services.Qualifications:

- Passing all background and requirements for facility and position; and

- Must have a valid and current driver's license and auto insurance.
Preferred Qualifications:
Bachelor's Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field, AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency experience in the education or treatment of persons with developmental disabilities or related conditionsBenefits:
