Attending bi-annual meetings with clients to monitor services Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications Assessing client needs and ensuring continuity of care. Maintain regular contact with collaborative partners. Making referrals and authorizing services based on client needs and personal outcomes. Maintaining records and documenting all services provided. Excels at documenting all interventions, interactions, and relevant information to the Support Plan consistent with professional and organizational standards Must reside within the metro area To perform this job successfully an individual must be able to perform each essential duty satisfactorily. Passing all background and requirements for facility and position; and The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding. Must have a valid and current driver's license and auto insurance. Requirements for All Case Manager II positions: Valid driver's license, excellent driving record and current auto insurance Education and Experience Required for Targeted Case Manager II position: Bachelor's Degree in behavioral science or related field 2,000 hours of supervised experience working with adults with severe and persistent mental illness OR Master's Degree in behavioral science or related field. Preferred Qualifications for Targeted Case Manager II position: Experience providing direct services to adults with serious and persistent mental illness Experience with case management and commitment proceedings Knowledge of Recovery, Motivational Interviewing and Person-Centered approach Education and Experience Required for Hennepin and Dakota County & Saint Louis County Waiver Case Manager II positions: Bachelor's Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field; OR Bachelor's Degree from an accredited four-year college with a major in any field, AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency Education and Experience Required for Ramsey County Waiver Case Manager II positions: Bachelor's Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND One year of experience in the education or treatment of persons with developmental disabilities or related conditionsBenefits:
