POSITION PURPOSE
The Case Manager is responsible for coordination of person-centered services for vulnerable individuals and assisting them in navigating complex service delivery systems. The Case Manager assists persons in gaining access to needed social, medical, educational and other supports and services. The case manager works on behalf of the person to identify their unique needs and to minimize the impact of the disability on the person's life while assuring continuity of services and supports for the person.
NATURE AND SCOPE
The Case Manager reports to the Lead Case Manager who reports to the Executive Vice President who reports to the President. The Case Manager is responsible for a variety of individual tasks, which require knowledge of disabilities and related conditions. Excellent interpersonal relationship skills are required. The job requires frequent contacts with clients, families, guardians, legal representatives, and other members of the interdisciplinary team to coordinate services for vulnerable individuals and assisting them in navigating the complex service delivery systems. The work environment requires frequent walking, standing, and sitting. Valid driver's license and ability to drive is required. The Case Manager participates in frequent phone, deskwork and computer work. This is a non-exempt position.
JOB RESPONSIBILITIES
CLIENT
SERVICES
  • Assists clients in locating and gaining access to necessary services as well as medical, social, educational, or other needed services regardless of the funding source; ensures informed choice for all individuals; coordinates person-centered plan of care using person-centered principles that balances what is important to a person with what is important for a person based on their needs and preferences; monitors services so the client's health and safety are assured; conducts reassessments; and conducts face to face visits every three (3) to six (6) months.
Assists in the identification of potential service providers; plans for arranges services and informs county waiver contact of proposed DD waiver services to be authorized, for entry into MMIS. Responds to all Minnesota Adult Abuse Reporting Center (MAARC) reports in a timely manner of receipt of report, documenting all responses and available for review upon request. Sits and writes and/or types frequently in order to prepare reports, forms, or information related to overall plan of care.
CUSTOMER/HUMAN RELATIONS
  • Functions as a role model, communicates effectively with clients, families, co-workers and outside agencies.
Ability to work with clients with a variety of cultural, medical, psychiatric, and physical needs. Displays positive, supportive, and cooperative conduct. Maintains and fosters cordial relationships and optimal communication. Acts as a role model in the appropriate conduct for meeting client needs. Communicates effectively verbally and in writing; listens accurately and responds appropriately.
REGULATION COMPLIANCE
  • Adheres to and maintains knowledge of various rule, regulations, and state and county policies regarding the DD waiver and Rule 185 Targeted Case Management. Completes all necessary documentation. Develops systems that provide continual improvement in quality services. SAFETY
  • Monitors and Responds to emergencies according to established policies and guidelines.
Maintains an injury free workplace by identifying and reporting unsafe working conditions.
DEVELOPMENT OF POSITION
  • Participates in on-going education in topics regarding the DD waiver and Rule 185 Targeted Case Management.
Attends and completes all annual in-service requirements.
ASSUMES ADDITIONAL RESPONSIBILITIES
  • Identifies and responds to client needs in emotional and social areas, however, intervention in these areas does not fall within the meaning of psychotherapy.
Performs tasks which may fall within Category II of infection control; there is no regular exposure to blood or high-risk body fluids.
QUALIFICATIONS
Bachelor's Degree in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of persons with developmental disabilities One (1) year experience in the education or treatment of persons with developmental disabilities or related conditions Excellent communication and interpersonal skills Excellent problem solving and organizational skills Telephone, computer, and communication skills Ability to read, write and speak English Ability to see and hear adequately to respond to emergency situations. Ability to travel to meet with clients and attend meetings, and if driving, holds a valid drivers license and maintains a safe driving record. Ability to sit, stand, reach, and bend.
