(PSCI) is looking to welcome a new Case Manager/QIDP to our Prairieland family in Decatur, IL , serving individuals and families in Champaign county . This position offers a consistent weekday schedule, excellent benefits, and the flexibility to work partially from home while making a meaningful impact in the lives of individuals with developmental disabilities. What You'll Do Assess individual needs and coordinate appropriate services and supports. Complete the PAS process in compliance with PAS Manual requirements Conduct required visits, Discovery, and Personal Plan development Advocate for individuals and families to access eligible and preferred services Review and monitor medical, residential, vocational, educational, legal, and financial needs Provide problem-solving consultation with provider agencies in accordance with DHS Conflict Resolution guideline Maintain accurate documentation, complete daily billings, and ensure compliance with agency guidelines Participate in staff meetings and required trainings Communicate trends, concerns, or service gaps to the Program Manager Required Qualifications Bachelor's degree in a human services-related field One year of cumulative experience providing direct services to individuals with developmental disabilities Experience may be gained through employment, volunteer work, or family caregiving What We're Looking For Strong organizational, time management, and documentation skills Knowledge of the developmental disabilities service system Ability to work independently while also collaborating effectively with a team Professional written and verbal communication skills Commitment to ethical standards and person-centered services Flexibility to work outside of normal business hours as needed Reliable transportation, a valid Illinois driver's license, and current auto insurance Why Join PSCI?
