Job description: De Novo Services is a small-but growing organization specializing in case management services across Washington State. Although we are in the process of expanding, our primary service areas are housing and employment. Job Summary We are seeking a full-time dedicated and skilled hybrid (primarily remote)

Case Manager to join our team . The
FCS/GOSH
Case Manager will be assigned a limited case load (less than 20 clients per PT staff member), with the goal of providing frequent contact to our clients who are enrolled in the Foundational Community Supports (FCS) or the Governor's Opportunity for Supportive Housing program. The GOSH Case Manager will be responsible for assisting individuals with disabilities in securing and maintaining meaningful employment opportunities and achieving housing stabilization. The position entails meeting with clients telephonically, virtually, and in-person, creating action/goal plans, utilizing available resources to help clients achieve their goals, conducting regular weekly check-ins with clients, managing client case files, and meeting with team members to provide updates. Case Manager will work closely with clients, families, and other professionals to ensure the delivery of high-quality care and support. This position is for Olympia, WA. The candidate must be comfortable with traveling within 40 miles of this region 2-3 times per week.
Duties Client Assessment and Goal Setting:
Conduct comprehensive assessments to understand the strengths, abilities, and goals of individuals with disabilities. Collaborate with clients to develop personalized employment plans, setting achievable goals for skill development and job placement. Collaborate with clients to develop personalized housing plans, setting achievable goals for securing and maintaining stable housing.
Resource Coordination:
Establish and maintain relationships with local businesses to identify suitable employment opportunities. Actively promote clients to potential employers and facilitate the job placement process. Provide guidance on resume development, interview skills, and other job seeking strategies Identify and connect clients with available housing resources, rental assistance programs, and supportive services. Establish and maintain relationships with landlords, property managers, and housing agencies to facilitate housing placements.
Ongoing Support:
Offer continuous support to clients in their new work environments, addressing any challenges or concerns that may arise. Work closely with employers to ensure a positive and inclusive workplace for individuals with disabilities. Implement strategies to enhance job retention and career advancement.
Advocacy and Education:
Advocate for the rights and needs of individuals with disabilities in the workplace. Educate employers, coworkers, and the community on the benefits of inclusive employment practices.
Documentation and Reporting:
Maintain accurate and up-to-date case records, documenting client progress, achievements, and challenges. Offer support in home community care settings as required. Qualifications Applicant must have at least 2 years of supportive housing experience. Bachelors Degree preferred. Applicant must have reliable transportation and must be willing to commute to meet clients. If you are not willing to do this, you will not be a good fit. Applicant must pass background check. Exemplary written communication skills. A self-starter attitude is absolutely necessary. Previous experience in employment services, case management, or a related field preferred. Strong communication and interpersonal skills. Ability to work independently and as part of a team. This is not an entry-level position. We will expect you to hit the ground running. You will have clients ready and waiting for you if you are offered a position. Attributes Empathy and understanding of the unique challenges faced by individuals with disabilities. Strong organizational skills and attention to detail. Flexibility and adaptability in a dynamic work environment.
Schedule Scheduling:
Although we operate between the hours of 7am and 7pm, case managers are free to choose the hours that work best for them, provided their schedule and availability is consistent and suits the availability of their clientele. Full-time employees expected to work minimum 40 hours per week.
Hybrid/Travel:
The

Case Manager will need to travel to clients in certain situations (client lives in Adult Family Home, is unable to ambulate, etc.). We expect that the
FCS/GOSH
Case Manager can expect to travel to clients one to two times a week. Case Manager may WFH the days they are not meeting with clients. Compensation & Benefits At De Novo Services, we embody a "work hard, play hard" approach. Schedules are flexible so long as the work is done and is done WELL.
