Responsibilities Coordinate case management activities for Medicaid Long Term Care and Comprehensive Program members Conduct comprehensive member assessments using care management tools and available clinical data Develop, implement, and monitor care plan activities to support positive health outcomes Collaborate with PCPs, providers, and community resources to support continuity of care Assist with prior authorizations, medication review, condition management, and service planning Use motivational interviewing and coaching strategies to improve engagement and wellness outcomes Maintain accurate documentation and follow regulatory and company guidelines

Requirements Required Experience:

Fluent bilingual Spanish/English required (Speaking, Reading, and Writing) Bachelor's degree required in Social Work, or related field At least 1 year of case management experience required Ability to travel 50-75% locally for face-to-face member visits in homes, assisted living facilities, and nursing homes

Preferred Experience:

Strong computer skills (Microsoft Teams, Word, & Excel) Long Term Care Experience/Managed Care Experience preferred Discharge Planning Experience preferred
