Job Description

Job Summary:
Innerlight Community Advocates is seeking a skilled and compassionate Houston Area Case Manager/QMHP-CS to provide case management and skills training services. This role involves working directly with minor and adult clients in their homes, schools, or other community settings to deliver trauma-informed and client-centered care. The ideal candidate will have a strong background in providing wraparound mental health services, with a commitment to empowering individuals to achieve independence and wellness. Candidate must be able to provide in-person case management services in Houston communities and neighborhoods.
Key Responsibilities:
Deliver case management services, including resource coordination and referrals for housing, employment, and other social services. Provide skills training to clients, focusing on coping strategies, emotional regulation, and life skills to support long-term recovery. Collaborate with other providers, including therapists, psychiatrists, and case managers, to ensure holistic care for clients. Maintain timely and accurate documentation of services provided in accordance with state and organizational guidelines. Participate in multidisciplinary team meetings to monitor and adjust client care plans.
Qualifications:
CANS 3-6, CANS
6-17, and
ANSA 2.0
certifications. Nurtering Parenting certification. Eligibility as a Qualified Mental Health Professional-Community Services (QMHP-CS) in Texas. Bachelor's degree, preferably in a social services counseling related field. Knowledge of trauma-informed care and evidence-based practices for treating mental health disorders. Strong interpersonal and communication skills with the ability to build trust with diverse client populations. Proficiency in clinical documentation and record-keeping. Valid driver's license and reliable transportation for community-based visits.
Preferred Skills:
. Experience working with Medicaid populations or underserved communities. Familiarity with co-occurring mental health and substance use disorders.
