LTC Case Manager Summary:

We're seeking a healthcare case management professional (1+ year required) to coordinate care for Medicaid Long Term Care/Comprehensive Program members through phone and in-person field visits. This role completes member assessments, builds and monitors care plans, coordinates services with PCPs and providers (including authorizations and community resources), escalates quality-of-care concerns, and maintains accurate case documentation to support member wellness and outcomes. Long Term Care Case Manager Requirements Fluent bilingual Spanish/English required (Speaking, Reading, and Writing) Long Term Care Experience/Managed Care Experience Discharge Planning Experience Bachelor's degree required in Social Work, or related field Ability to travel 50-75% locally for face-to-face member visits in homes, assisted living facilities, and nursing homes Strong computer skills (Microsoft Teams, Word, & Excel)
