Job Description

Every TAP employee shares in TAP's responsibility to "mobilize community resources" in support of TAP's anti-poverty efforts.
DEFINITION AND PURPOSE
: The Mental Health Navigator is employed to provide services that include connecting Virginia
CARES WINGS
clients to community services and supports for mental health, health care benefits or community health care services. The Mental Health Navigator provides case management and care coordination, mental education, interdisciplinary collaboration, coordination, consultation, and administrative duties. The Navigator works closely with the Virginia Cares case management staff to develop comprehensive plans for the success of female clients. The Coordinator works collaboratively with the team and the female client to identify and address systems challenges for enhanced care coordination as needed.

RESPONSIBILITIES
: Conduct assessments to understand the clients' situation, potential barriers to care, the causes, and the impact of such barriers on the clients' ability to access and maintain supportive services. Develop the mental health care plans with an interdisciplinary team for clients and focuses on the system navigation to community services and referrals needed for the clients. Follows-up with clients and referral partners to ensure that the clients have connected, evaluates the effectiveness of the services and supports, and makes appropriate modifications to ensure the provision of high-quality care and interventions. Monitor the clients' progress, maintains comprehensive documentation, and provides information to case managers when appropriate. Modify services to meet the needs of clients and coordinates services with other organizations and programs to assure the services are complementary and comprehensive. Serves as the liaison to community health care programs and represents the WINGS program in contacts with other agencies and the public. Coordinates supportive and additional services with the clients and works with case managers to link the clients to supportive services, which include, but are not limited to, mental health support, substance use treatment, transitional housing, transportation, and child care. Serves as the subject matter expert on community resources related to the needs of clients and is responsible for educating clients of the available services and coordinating the appropriate referrals to the resources based on the clients' preferences. Determine the needs, strengths, limitations, and preferences of each WINGS client and will engage in problem-solving to identify and reduce barriers to mental health care, substance use treatment, and family engagement. Monitors the clients' progress toward meeting goals for mental health support, substance use and treatment, and family engagement and promotes progress towards health and well-being for the clients. Adheres to professional boundaries and ethical principles about confidentiality, informed consent, compliance with relevant laws, and agency policies (e.g. HIPPA) Identifies systemic barriers for access to services, communicates with team and leaderships about the barriers, and works collaboratively to find viable solutions. Assists in developing policy, procedures, and practice guidelines related to the WINGS program using knowledge gained from research and best practices. Builds relationships with mental health support groups, substance use and treatment organizations, family and child engagement networks, and other referral networks. Assists with mandatory reports required by funding source, supervisor and Director for This Valley Works. Performs other duties as assigned and required.
TRAINING, EDUCATION AND EXPERIENCE
Degree in social work or equivalent education/experience is preferred. Experience working with individuals challenged by incarceration history, substance abuse, and/or mental health issues. Experience working in a collaborative multi-agency partnership environment. Enthusiastic and able to motivate partners and participants. § At TAP, we focus on the Whole Family Approach. ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES
Participate effectively in team meetings, case conferences, and related activities. Collaborates with an interdisciplinary team in a manner that enhances the coordination of comprehensive care for clients Effectively communicate with and utilizes community agencies to facilitate continuity of care. Maintain regular contact and interaction with a variety of community agencies and resources and collaborates with a variety of community agencies and engages in problem resolution activities. Knowledgeable of local resources and service linkages. Ability to establish, implement, and maintain effective working relationships with other agencies and the general public. Effective oral and written communication skills. Must be able to relate to person of varying socio-economic backgrounds. Must have functional knowledge and experience in the use of computers for correspondence, compilation of reports, and management of budgets. Ability to supervise the preparation of complex statistical and financial reports.
SPECIAL REQUIREMENTS
Valid Virginia Operator's license with a good driving record and reliable transportation. § TAP participates in the Virginia DMV Driver Alert Program. Employees who operate a TAP vehicle or their personal vehicle to conduct TAP business may be subject to driver record monitoring through the Virginia DMV Driver Alert Program. This position is eligible for remote work upon supervisor's approval. Must pass drug screening. Some positions require positive complete background screening including: criminal, sex offender and child protective services.
