Human Services Social Work Full Job Description Description:
Every veteran deserves the chance to age or heal on their own terms, in their own home. At LOA, that's exactly what you'll help make possible. What we're looking for : a
Veterans Directed Care (VDC) Options Counselor
to be a trusted guide for veterans navigating self-directed, in-home care.
You'll be part case manager, part advisor, and part advocate: assessing needs, building individualized spending plans, and coaching veterans through the ins and outs of managing their own care team, all while keeping programs compliant and veterans supported. This isn't a desk job with a script. It's a role for someone who can walk into a veteran's living room, earn their trust, and walk out with a plan that actually works for their life. What you'll do :
Conduct in-home assessments and reassessments to determine eligibility, needs, and risk
Develop and oversee individualized spending plans in line with VA and VDC guidelines
Coach veterans through their responsibilities as employers ( hiring, training, and supervising their own care workers)
Monitor spending and outcomes, flag compliance risks, and keep documentation audit-ready
Represent LOA with VA partners, community agencies, and stakeholders What you'll bring :
Bachelor's degree in Social Work, Gerontology, Community Organization, or related field (preferred)
5+ years of progressively responsible experience in program coordination, compliance, counseling, or human services
Sharp independent judgment who can manage a complex caseload
Strong writing and reporting skills, and the people skills to mediate hard conversations
A valid Virginia driver's license and the willingness to travel across Virginia and bordering states
Why LOA:
You'll have autonomy, a mission that matters, and the satisfaction of knowing your work keeps veterans out of nursing homes and in the communities they fought for.

$55,000.00 per year
