• Conduct strengths-based, person-centered assessments for adults with developmental disabilities
• Meet with individuals in their homes and community settings
• Interview family members, caregivers, and support staff to gather information
• Evaluate functional abilities and support needs
• Complete assessment documentation and submit accurate coding within required timelines
• Manage your own schedule while meeting quality and productivity goals Education 🎓 Bachelor's Degree in a Human Services, Health, Behavioral Science, or related field (Degree requirement cannot be waived.) Experience ⭐ At least 1 year of post-graduate professional experience working directly with adults who have intellectual and/or developmental disabilities Examples include:
• Case Management
• Care Coordination
• Social Work
• Developmental Disabilities Services
• OPWDD Programs
• Behavioral Health

Pay Details:
