12:00 PM - 5:00 PM One (1) Flexible 8-hour day of the employee's choosing (subject to supervisor approval and program needs)
Compensation Hourly Rate:
$16 per hour Position Summary The Community Case Manager plays a key role in supporting individuals and families connected to the Neighborhood Center by providing compassionate, organized, and solutions-focused case management services. This position focuses on intake assessments, identifying family strengths and needs, connecting families to community resources, monitoring progress toward goals, and strengthening partnerships throughout the community. The flexible workday is designated for outreach efforts, attendance at community events, partnership development, and collaboration with local service providers to expand opportunities for participating families. Key Responsibilities Family Intake & Assessment Conduct intake interviews and family needs assessments for new participants. Identify immediate needs, barriers, strengths, and priority concerns. Develop individualized Family Goal Plans in collaboration with families. Maintain accurate participant files and confidential records. Case Management Services Provide ongoing support, guidance, and encouragement to enrolled families. Monitor progress toward benchmarks outlined in Family Goal Plans. Conduct regular follow-up calls, check-ins, and progress reviews. Assist families with problem-solving and navigating available services. Referrals & Resource Coordination Connect families to housing, employment, food assistance, counseling, healthcare, education, transportation, childcare, and other community resources. Ensure all referrals are completed and followed up on in a timely manner. Track outcomes of referrals and address barriers to access. Build and maintain an updated resource directory. Community Engagement & Partnerships Use flex day to attend community events, networking meetings, and collaborative initiatives. Represent the Neighborhood Center professionally in the community. Build relationships with schools, nonprofits, faith-based organizations, and agencies. Identify new resources and partnership opportunities that benefit families. Documentation & Reporting Maintain timely case notes, progress reports, and service documentation. Submit weekly activity summaries and outcome updates. Ensure compliance with confidentiality policies and agency standards. Assist with data collection for grants and program reporting. Qualifications Required Associate degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, or related field or equivalent experience. Minimum 1 year of experience in case management, family services, community outreach, or related human services work. Strong communication, organization, and relationship-building skills. Ability to work independently and manage a flexible schedule. Proficiency in Microsoft Office / Google Workspace. Reliable transportation for community outreach and events.
