LoveLynn is seeking a compassionate and resourceful Case Manager/Housing Specialist to provide individualized case management services to a small caseload of approximately (6) Veterans experiencing homelessness that is currently in temporary housing. The primary focus of this position is helping Veterans successfully transition from temporary housing into safe, stable, permanent housing.

Caseload:

The Case Manager will maintain a small caseload of approximately two Veterans at a time. As Veterans successfully transition into permanent housing, new Veterans will be referred into the program, creating an ongoing caseload throughout the contract period for 5 years. Responsibilities Develop an individualized service and permanent housing plan for each Veteran. Identify apartments, landlords, housing programs, and other permanent housing opportunities. Assist Veterans with housing applications, required documentation, and communication with landlords. Connect Veterans with VA benefits, HUD-VASH, community housing resources, and other eligible programs. Assist with budgeting, financial planning, employment resources, and independent living skills. Connect Veterans with medical, mental health, substance use, and community resources as needed. Maintain regular communication with VA case managers and other service providers. Document services, Veteran progress, housing barriers, referrals, and outcomes. Advocate for Veterans and help resolve barriers that may delay permanent housing placement. Conduct regular follow-up and case management meetings with each Veteran. Qualifications Experience in case management, housing navigation, social services, behavioral health, homelessness services, or a related field preferred. Experience working with Veterans or individuals experiencing homelessness is preferred. Knowledge of

HUD-VASH, VA

resources, affordable housing, or community housing programs is a plus. Strong communication, organization, documentation, and problem-solving skills. Ability to work independently and coordinate with multiple community providers. Valid driver's license and reliable transportation.

Help each Veteran overcome barriers to housing and successfully transition into permanent housing as quickly and safely as possible.
