PSH Case Manager/PRP Job description (Full-time)
RESPONSIBILITIES
Collaborates with client in the formulation of a Case Management Plan that is reviewed and modified with client on a regular basis; Provides outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, apartments, office, etc; Provides individually tailored services to each client in the following areas: housing, school, training & work opportunities, activities of daily living, health, wellness self-management, relapse prevention, money management & entitlements, medication support, self help & empowerment, problem solving, family life & social relationships and integrated treatment for substance abuse; Maintains written and computerized records, compile reports and completes other program documentation (e.g. progress notes, case management plans, intakes, biopsychosocial assessments, monthly visit reports, incident reports); A minimum 2 contacts per month for housed clients, including 1 face to face contact in the client's apartment; additional monthly contacts as determined by the Case Management Plan and assessment of client needs; Coordinates and monitors referrals to community services and advocates client participation; Participates in weekly staff meetings; Participates in staff trainings as scheduled; Performs related work as assigned; Additional Responsibilities for PRP clients: Create and implement rehabilitation plans PRP participants receive 6 contacts a month. Collaborate with other mental health providers
QUALIFICATIONS
Bachelor's degree in social work, psychology, sociology, counseling, or related social service/science or healthcare related disciplines and one (1) year of experience providing case management services or equivalent specialized training and/or field placement experience. Certification and/or licensure in a relevant discipline (e.g., certified addictions counselor) may substitute for educational requirements. In lieu of the education, licensure or certification, the Case Manager shall have a minimum four (4) years of experience with marginalized populations. Additional considerations can be made on a case-by-case basis for applicants with a previous history of homelessness. Excellent organizational, communication, (both oral and written) skills preferred. Ability to work independently is strongly desired. Excellent computer skills highly desired Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations; Bilingual English/Spanish is strongly desired. Driver's License and access to personal vehicle required;
