What You'll Do:

As a QIDP, you'll partner directly with the people we serve, their families, and support teams to create and guide individualized service plans. You'll lead Community Support Team (CST) meetings, coordinate care, and ensure services align with each person's goals and preferences. This role blends office work with hands-on time in homes and the community—making a real impact every day.

Schedule & Expectations:

Full-time (minimum80 hoursper pay period) Typical hours:8:30 a.m. -5:00 p.m. with flexibility Approximately 25% of time spent in the field Somepre-plannedevening/weekend hours and rotating on-call responsibilities

What You'll Need:

Bachelor's degree in Social Work, Psychology, Special Education, or related field At least 1 year of experience supporting individuals with intellectual/developmental disabilities Eligibility for (or current listing on) the

DHS/DDD QIDP
