Job Description

A community-based organization on Long Island is seeking a Service Coordinator for a hybrid role in Amityville. This position provides case management, housing stabilization support, and community-based coordination to help participants work toward long-term housing stability.  About the

Responsibilities:
Provide short-term intensive case management for veteran households. Develop and monitor comprehensive housing stability plans based on participant strengths, needs, abilities, and preferences. Conduct assessments, maintain regular contact, and track participant progress toward housing and service goals. Assist participants with housing search and placement, benefits access, and referrals to community supports. Complete documentation and reporting in required systems, including HMIS, while coordinating with the broader service team.
Qualifications:
Bachelor's degree in Human Services or a related field, or a combination of education and experience. At least 1 year of experience in social services. At least 3 years of experience in case management, homelessness prevention, rapid rehousing, or transitional supportive services. Proficiency with Microsoft Windows, Word, Excel, Outlook, and Teams. Strong communication and writing skills. Ability to work in a hybrid setting and in the field as needed. Reliable transportation for community-based work.
Desired Skills:
Experience with intake, community outreach, or electronic database systems such as AWARDS. Background supporting homeless families or veterans. Knowledge of housing-first, rapid rehousing, and homeless prevention services. Strong organizational skills and attention to detail.
