Service Coordinator/Case Manager

unBefuddled provides Service Coordination / Case Management for adults with Developmental Disabilities throughout Idaho . We link our clients to services, supports, and resources to encourage independence, skill-building, and community access. Our mission has always been the same: to effectively care for our team, our community and the people that we support. unBefuddled strives to provide quality case management by helping people understand what their choices are and connecting them to resources in their community, while respecting their dignity and rights. The perfect service coordinator for unBefuddled is an individual who is self-motivated and an independent worker. Candidates will be proficient writers in the English language, basic computer skills, be successful at managing their own work schedule, and have the ability to work remotely. All applicants in Idaho or near an Idaho border are encouraged to apply! This position is suited for someone who is interested in starting the position part time. Position can stay part time or grow to full time.

Job Duties:

Successfully manage a caseload of clients and meet deadlines for service delivery. Advocate for clients needs. Facilitate meeting with your client and their care team. Link participants to community resources, vocational programs, developmental services, medical providers and mental health services. Coordinate and monitor developmental services. Keep detailed and accurate record of activities to maintain compliance with Medicaid. Create and maintain professional relationships with other service providers and agencies in Idaho.

Candidate Requirements:

Bachelor's degree in a human services field(ex: Psychology, Sociology, Child Development, Counseling, Education, Nursing, etc) Pass a criminal background check (certain felony convictions harming another person will exclude candidate from hire, per Medicaid rule) Have reliable transportation to visit clients and attend appointments (you will not transport clients)
