Job Description
North East Ohio Network (NEON) is a county council of governments agency dedicated to supporting County Boards of Developmental Disabilities throughout Northeast Ohio. NEON serves children and adults who are at risk for or have a diagnosed developmental disability, helping individuals access the services, supports, and opportunities they need to live meaningful, fulfilling lives. We are seeking a full-time Service & Support Administrator (SSA) to join our team. Make a Difference Every Day As an SSA, you have the opportunity to make a meaningful difference in the lives of the individuals you support. Your role is to help each person identify and achieve the outcomes that are important to them while ensuring they have access to appropriate services, resources, and community opportunities. A typical day may include meeting with individuals in the community, coordinating with families and service providers, attending planning meetings, connecting individuals with community resources, addressing concerns, and completing case notes and updates to individual planning documents. We provide comprehensive training for new hires. Previous experience in case management, service coordination, developmental disabilities, or mental health is preferred, but we welcome candidates who have the education, interpersonal skills, compassion, and commitment needed to succeed in this role. What You'll Do As an SSA, you will: Serve as the primary point of coordination for each individual's person-centered planning team. Travel to various counties throughout the NEON region to provide SSA services for County Boards experiencing SSA staffing shortages, including covering assigned periods as needed. Meet with individuals and their teams to identify needs, areas of potential risk, and outcomes that are important to and for the individual. Complete and/or coordinate comprehensive assessments to determine the need for services and supports, revising assessments as needs change and completing required annual updates. Facilitate the development and ongoing review of person-centered Individualized Service Plans (ISPs), including identifying and coordinating needed supports. Identify appropriate funding sources for services and supports using the applicable hierarchy of funding options. Establish and maintain relationships with service providers, family members, and natural supports to ensure services are effectively coordinated. Promote community presence, participation, independence, and inclusion. Provide referrals and connections to community resources that help individuals meet their needs and pursue their goals. Monitor services on an ongoing basis to ensure supports are delivered according to each individual's plan and that progress is being made toward desired outcomes. Take appropriate action when concerns arise regarding an individual's health, safety, or welfare. Assist individuals in making informed choices regarding service providers. Establish eligibility for County Board of Developmental Disabilities services. Assist individuals with enrollment in Medicaid waivers when appropriate. Maintain timely and accurate documentation, case notes, assessments, plans, and other required records. Comply with Ohio Department of Developmental Disabilities (DODD) and County Board requirements regarding the reporting of abuse, neglect, and other incidents, including Major Unusual Incidents (MUIs). What We're Looking For The ideal candidate is compassionate, organized, dependable, and comfortable working independently. Strong communication and relationship-building skills are essential, as SSAs work with individuals, families, providers, County Board staff, and community partners.
