Role:

Amazing Strides is seeking a Part-Time Support Coordinator / Case Manager to support individuals with developmental disabilities through New Jersey's DDD (Division of Developmental Disabilities) program. This role starts as part-time with growth opportunities.

Key Responsibilities:

Develop and manage individualized service plans (ISP) Coordinate services and providers under DDD Conduct monthly and quarterly monitoring visits Support families navigating Medicaid and DDD systems Maintain compliance documentation and case notes

Qualifications:

Bachelor's Degree required Experience in case management, DDD, Medicaid, or human services preferred Strong communication and organizational skills Reliable transportation (field-based role)
