Person-Centered Planning:

Work with individuals to develop a Person-Centered Support Plan (PCSP) that aligns with their goals, preferences, and what is important to them.

Service Access:

Assist individuals in accessing and receiving a range of needed services and supports, including day programs, residential care, and employment support, as outlined in the IDD Waiver.

Advocacy:

Act as an advocate for individuals to ensure they receive the appropriate services and supports that best meet their needs and help them achieve their goals.

Service Monitoring:

Monitor and evaluate the services in place to ensure they are meeting the client's needs and effectively supporting their preferred lifestyle.

Community Resource Connection:

Connect individuals with community resources beyond the waiver to promote independent living and community involvement.

Coordination:

Coordinate various aspects of care with different service providers to improve continuity of care and overall outcomes for the individual.
