Key Responsibilities Coordinate case management services for Medicaid Long-Term Care members Conduct comprehensive member assessments using care management tools and data review Develop, implement, and monitor individualized care plans Collaborate with interdisciplinary care teams to achieve optimal health outcomes Track progress and update documentation in accordance with program requirements Travel Requirements Up to 75% field-based travel within Miami-Dade County Travel time paid at hourly wage Mileage reimbursed at IRS rate Reliable transportation required Required Qualifications Case management experience required Bachelor's degree in Social Work or a related Human Services field Candidates must reside in Miami Dade County, FL (MUST
