We are currently seeking a Transition Coordinator to join our Transitional Services program. This is a meaningful opportunity for professionals who are passionate about helping others live healthier, more independent lives. As a Transition Coordinator , you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You'll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community. Key Responsibilities Act as point-of-contact to assist the individual to live in his/her home and/or community. Assist individuals served with linkages to service providers and community resources as needed. Identify barriers to compliance with individual's healthcare regime, appointments and screenings. Work with individuals to create solutions to identified barriers and empower individuals to manage physician, and/or other key medical professionals, contacts, appointments and/or screenings. Communicate and collaborate with the individual's healthcare team, including, but not limited to, the facility Social Worker and Waiver Case Manager. Adheres to the CareStar Rule in performance of job responsibilities. Understands and complies with CareStar Policies and Procedures. Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense. Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc. Minimum Qualifications At least 18 years of age or older. High School Diploma or higher education level. Minimum of 12 months of experience delivering services in a healthcare, behavioral health or social services setting. Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities. Why Join CareStar?
