Youth-Family Advisor (New Castle County)

ROLE AND RESPONSIBILITIES

The duties listed below are intended as examples of the types of work that may be performed by a Youth-Family Advisor, but are not necessarily performed and do not include all specific functions and responsibilities that the Youth-Family Advisor may be expected to perform. • Provide Youth Reentry Transition Support services during the assigned youth's out-of-home placement and continue support for the youth and family for up to one year after discharge. Conduct needs assessments with each youth and their family, create individualized reentry plans, and provide ongoing case management at intake and regular intervals while the case remains active. Facilitate connections to necessary resources and supports before the youth's release and for up to one year post-discharge; provide wrap-around case management to support resource utilization. Coordinate services and actively place youth and families at relevant job training programs, housing resources, employment opportunities, and social services, including substance use and mental health treatment programs. Plan and facilitate workshop sessions for youth in residential placement settings; workshop topics will include job readiness and soft skills, peer and family relationships, restorative justice, and other essential life skills. Provide emotional support and assist participants with navigating the challenges and successes of their reentry transition; promote family strengths and connection to supports. Maintain regular contact with the Community Support Specialist regarding service coordination and attend recreation and group community activities. Provide regular, face-to-face contact at the residential facilities, in the home, at school, or in the community. Maintain frequent follow-up with youth and families. Collaborate with YRS treatment teams and families to support and create an individual reentry plan; attend monthly treatment meetings, aftercare, and other planning meetings. Assess and report on client progress and participation at regular intervals; Track and update client information using the agency's database program; complete case notes and maintain regular contact with YRS regarding youth progress. Assist with policy, communications, and development as they relate to the program. Accept and perform other work as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

Understands and supports the Delaware Center for Justice and Restorative Justice Philosophy and mission. Significant knowledge and experience related to justice-involved people, the social forces and systems impacting them, and the resources available to them are required. A Bachelor's Degree in human services, social work, or a related field is also required. Master's degree preferred. Experience in motivational interviewing, strengths-based case management, trauma-informed practice, and/or Cognitive Behavioral practices also preferred. Background checks and clearances required as per program contract(s). Ability to multitask and effectively manage work at different sites (in-office and off-site). Must have a valid Delaware Driver's License; Use of personal vehicle required. Strong team building, organizing, writing, and other communication skills are essential. Experience with group facilitation preferred. Basic computer skills, including but not limited to the use of Microsoft Office, as well as Zoom/Skype and other distance meeting resources. Ability to multitask and work both independently and with supervision, on-site and remotely. Embrace an understanding of justice involvement that acknowledges the impact of institutional racism and the systemic oppression of BIPOC, women, and LGBTQ+ people; must share the belief that people can change in positive ways. Must be comfortable providing community-based services. Must embrace teamwork, have a strong work ethic, be a self-starter with a positive attitude, with a good sense of humor.
