Job Description

HomeAgain, a Richmond-based nonprofit serving the needs of those seeking shelter and long-term housing, is seeking a skilled full-time, mission-focused Community Case Manager (CM). The Community CM assist clients to develop goals and objectives related to obtaining and maintaining permanent housing and increase self-support. Responsibilities include assessment of client needs, referrals to community resources, ongoing client contact to provide support and evaluate progress and the administration of financial rental assistance. The CM will maintain a caseload that includes single adults and families with children at HomeAgain comprised of clients enrolled in rapid rehousing.
Responsibilities include:
Operates as part of a Community Housing Team to facilitate and ensure cohesion among program staff and consistent implementation of program procedures. Attend and participate in staff meetings, agency/community trainings, client reviews or other meetings to encourage client progress and provide positive interventions for all residents. Participate in on-call weekend responsibilities. Coordinate with Homeless Point of Entry, the CoC's Housing Teams and other community providers to maintain full capacity in the program. Notify Program Manager for Community Housing of needed services/resources for participants. Provide follow up services with clients housed through HomeAgain in their homes for a specified period of time. Demonstrate a positive role model for clients at all times. Complete other duties as assigned in a timely and efficient manner.
Client Service Responsibilities:
Provide intake and eligibility assessments for clients assigned by the Housing Teams and by HomeAgain. Ensure completion of necessary forms at client intake, complete social history assessment and all other required paperwork for client files. Determine eligibility for permanent housing programs and resources. Administer financial assistance in accordance with funding rules and regulations. Meet with clients on a regular basis to facilitate placement in permanent housing Utilize strengths perspective and positive working relationships with diverse clients, in order to remove barriers to housing and to facilitate client's movement to permanent housing. Transport clients as needed. Keep accurate records and make timely entries into HCIS Database and in HomeAgain files. Complete individualized client service plans with goals and timelines for completion. Enter service encounters into HCIS within 48 hours of providing the service.
