COMPENSATION:
$22.51/hr.
DOE WORK ENVIRONMENT
This position requires the employee to work onsite. Remote work is not supported.
RESPONSIBLE FOR
This is a full-time non-exempt position providing crisis support, advocacy, and comprehensive case management services for runaway and homeless youth. This position works to identify and support runaway and homeless youth through targeted outreach, crisis support, assessments, interventions, and referrals.
Case Managers general responsibilities are:
1) Develop and implement life skills programs for runaway, homeless, and at-risk youth. 2) Conduct case management with homeless youth in the Transitional Living Program (TLP) or Safe Shelter Program. 3) Assist with outreach and provide or connect youth to basic need supports (food, hygiene products, clothes), counseling, academic support, job assistance, case management, and enrichment activities.
ESSENTIAL DUTIES
Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses. Implements and participates in professional-level runaway homeless youth programs that requires contact with clients and community. Responsible for ensuring case management and shelter activities correlate with established goals and objectives. Requires completion of specialized training that meets Family Youth Service Bureau (FYSB) requirements, including but not limited to crisis intervention and de-escalation, human trafficking, suicide prevention, harm-reduction, and trauma-informed care. Conduct outreach regularly, going to places youth frequently congregate (schools, parks, skate parks, etc) to identify and connect with youth in-need and promote Youth Outreach services and programs. Identify and create lists of locations for street outreach where youth most commonly congregate. Conduct case management with homeless youth in the community and Transitional Living Program (TLP) age 18-24 and Safe Shelter Program (11-17). Provide case management to youth and support youth clients in development and achievement of goals. Conduct case management with homeless, runaway, and at-risk clients in Youth Outreach programs. Provide on-going support for runaway, homeless, and at-risk youth including developing and monitoring client goals, advocacy, referrals to community resources, and outreach. Refer youth to appropriate community resources and provide follow-up monitoring and support. Provide or connect youth to basic need supports (food, hygiene products, clothes), counseling, academic support, job assistance, case management, and enrichment activities. Maintain current documentation and records as required for program evaluation and reporting purposes and ensure all files and reports are accurate and meet grant standards. Develop and coordinate life skills classes such as budgeting, household activities, job readiness, anger management, social skills, and study habits. Serve as "after-hours" on-call staff as needed to ensure participant monitoring and timely crisis response. Use Teams calendar ensure communication with all staff. Provide follow-up monitoring on youth shelter placements or referrals. Collaborate and coordinate with other service providers as appropriate. Other duties as assigned by Program Director/Managers.
KNOWLEDGE, SKILLS AND ABILITIES
Bachelor's degree in Social Work or Related Field Preferred (or extensive work experience). Experience in personnel supervision and team leadership preferred. Strong interpersonal skills for connecting and working with youth from diverse backgrounds (including, but not limited to, ethnic, religious, physical and mental abilities, etc.). Strong oral and written communication skills. Familiarity with Trauma Informed Care, Healing Centered Engagement, and Critical Time Intervention. Commitment to work as an essential part of a team. Demonstrated ability in the implementation and development of quality youth programing. Ability to be flexible in work schedule. Ability to pass drug/alcohol & background check and insurance check of driving record. Experience in case management and providing direct services to at-risk youth, ages 11-21 and their families. An understanding of at-risk, runaway, homeless, and street youth issues. A familiarity with youth services in Yamhill County. Recognition of appropriate boundaries and the ability to implement them. Ability to work effectively in stressful or crisis situations. Commitment to empower youth to make informed decisions. Ability and willingness to work under pressure.
PHYSICAL DEMANDS
The physical demands described represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is occasionally required to stand; walk on uneven terrain; sit for extended periods of time; climb stairs/hills. The employee must occasionally lift or move up to 40 pounds. Employee will be subjected to outside environmental conditions while conducting various levels of outreach to youth. In addition to working in a typical office environment, the outreach worker will be required to work in areas frequented by youth.
Pay:
$22.51 per hour
