Specialty Case Manager (ACT Team)
Reports To:
Supportive Services Manager Purpose of Position :
Provides comprehensive case management services to the Providence Center Behavioral Health ACT clients experiencing homelessness or housing instability including assessment, case planning, and evaluation of services. Collaborates with treatment teams from partner agencies to ensure coordination of services and successful outcomes.
Duties and Responsibilities:
Advocates for the mission of the organization and promotes the vision and direction of the agency's values. Delivers services with safety, respect, and effectiveness. Follows and adheres to all policies and procedures of Crossroads and the Providence Center. Provides case management support to clients receiving services, including assisting clients with obtaining housing, connecting clients to mainstream services (health care, behavioral health services benefits/entitlements, etc.), assisting with budgeting and financial literacy needs, meaningful daily activities, vocational and educational development etc. Hands on support teaching ADLs (cooking, cleaning, hygiene, household chores, etc). Maintains a caseload of 10 -15 with the potential to flex up to 20 clients when coverage is needed. Uses client-centered, evidenced-based interventions when providing services to clients, including Trauma Informed Care, Harm Reduction, Progressive Engagement, and Motivational Interviewing. Completes initial and ongoing assessments with clients using standardized assessment tools such as the SPDAT. Uses assessment information to establish goals with clients, and to develop client-centered, trauma-informed Case Plans, which contain specific measurable objectives and timeframes for completion. Monitors signs and symptoms of psychiatric decompensation and completes basic risk assessments, escalating to appropriate crisis response (ES or QMHP) staff when indicated. Completes safety plan and risk minimization plan to appropriately support clients in the community. Provides supportive services, psychoeducation, and skills training to help clients find and maintain stable housing. Provides services in vivo, including home visits and accompaniment to appointments. Provides referrals to appropriate service providers within the agency and externally. Advocates for clients within Crossroads and with outside agencies, landlords, courts and other systems as needed. Reviews and evaluates client progress toward Case Plan goals on an ongoing basis and adjusts case plans as appropriate. Practices fidelity to the Housing First and Assertive Community Treatment models, ensuring that

standards are met working with service users to achieve greater independence over time. Attends Crossroads and the Providence Center team meetings and functions as part of the Providence Center's multidisciplinary team, attending all team meetings and collaborating with all members regarding client treatment. Maintains professional rapport with service user, peers, and other community service providers Completes recordkeeping and documentation, both paper and electronic, in accordance with Crossroads' policies and The Providence Center, and standards of professional practice. Adheres to TPC documentation requirements and ensures they are completed in a timely manner. Maintains confidentiality in accordance with agency policies, state regulations, HIPAA, and other relevant requirements. Performs other duties as assigned.
Requirements:
Knowledge, Skills and Abilities Familiarity with requirements of Intensive Case Management and Assertive Community Treatment models of service delivery. Excellent interpersonal skills and ability to engage and establish rapport with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and other conditions or situations that have impacted housing stability. Demonstrated ability to work collaboratively with diverse groups of people, including people experiencing homelessness, colleagues, professional groups and community agencies. Must be able to work a flexible schedule, including regularly scheduled evening hours. Excellent verbal and written communication skills. Demonstrated problem solving ability. Must be able to apply good judgment, agency values, and policies and procedures to guide decision-making. Ability to deal effectively with frequent changes, delays, or unexpected events in the work environment. Ability to maintain confidentiality in all aspects of the work environment. Must successfully complete training on the Housing First Model, Trauma Informed Care, Mental Health First Aid, Non-Violent Crisis Intervention (CPI Model) and Harm Reduction along with the Providence Centers CCBHC required trainings within 90 days of employment. Ability to maintain appropriate professional boundaries. Ability to maintain self-control and manage difficult or emotional situations using reason and the organization's values. Ability to motivate and organize self to complete tasks with service users, often in non-therapeutic environments. Strong computer skills including experience with Microsoft Office and Data Entry. Requires verbal and numerical intelligence. Requires transporting clients with the use of personal or CRI vehicles (depending on availability). Requires a valid driver's license and insurance.
Education and Experience:
Bachelor's degree in Human Services field OR Associates Degree in a Human Services field plus a minimum of 2 years experience in a Human Services field such as advocacy or case management OR A certification in Case Management plus a minimum of 2 years experience in a Human Services field such as advocacy or case management OR A minimum of 8 years experience in a Human Services field such as advocacy or case management.
