We deliver evidenced-based behavioral health services to children, youth and adults, and has long been at the forefront of culturally sensitive foster care and adoption services.
POSITION DESCRIPTION
The Case Manager provides counseling, case management, advocacy to help individuals and families overcome challenges related to abuse, addiction, poverty, and health. organizes, facilitates, and monitors care plans to support clients' health and social needs, assessing client needs, developing treatment plans and advocating for services and conducting home visits.
REQUIREMENTS & QUALIFICATIONS
Bachelor's degree in social work, behavioral science, or a related field. Experience with Trauma-Informed Care and Adverse Childhood Experiences (ACEs) is often preferred. Valid Tennessee motor vehicle operator's license. Successful completion of Tennessee Department of Children's Services training (for state positions). Effective communication with clients, families, and service providers. Managing multiple, complex cases simultaneously. Ability to navigate challenging situations while remaining empathetic. Organization and ability to maintain accurate records. Proficiency in case management software. Crisis intervention and problem-solving skills
DUTIES & RESPONSIBILITIES
All the responsibilities listed below are considered essential functions of this position. Additionally, this position is expected to ensure adherence to the core operating values and the strategic direction of the agency.
Assessment & Planning:
Identifying client needs, creating individualized service plans, and evaluating risks.
Coordination of Care:
Navigating systems of care and connecting individuals to community resources.
Advocacy & Support:
Serving as a dedicated advocate for clients' mental, physical, and emotional health.
Documentation & Monitoring:
Monitoring, evaluating, and documenting case progress, including testifying in court. Prepare families for visitation, home leave, aftercare, and group/family meetings Connect youth and families to appropriate community resources Participate in internal audits and reviews to ensure policy compliance Perform other duties and responsibilities that may be necessary or in the best interest of the agency as assigned by Supervisor Confidentiality and Privacy Adheres to all legal parameters for ensuring and maintaining clients' confidentiality including HIPAA, State and Federal laws. Physical Activity Reaching, Standing, Typing, Talking, Hearing, Repetitive Motion Sedentary work. Exerting up to 15 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. The worker is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading. The worker is not substantially exposed to adverse environmental conditions.
