The Case Manager (CM) exists to ensure all persons supported have the services and supports needed to have a meaningful day and to live their vision of valued community living. CM's monitor services and supports for quality and continuity. They are responsible for maintaining all documentation related to services, supports, and oversights to ensure all needs for persons supported are met. All duties are performed with the knowledge that Breakthrough strives to be the model of excellence for adults with autism.Job Type:
