Mobile Unit Case Manager

We are looking for a compassionate and driven Case Manager to join our Mobile Unit Team , delivering care directly into the community. This is not a traditional office role—you'll work alongside counselors on a mobile bus , meeting clients where they are and helping connect them to critical resources that change lives. What You'll Do As a Case Manager, you'll play a key role in supporting individuals through coordinated, client-centered care: Provide comprehensive case management services in a mobile, community-based setting Conduct intake assessments and develop individualized service plans Collaborate with counselors and team members to ensure holistic care delivery Connect clients to community resources (housing, healthcare, employment, social services) Monitor client progress and adjust service plans as needed Maintain accurate, timely, and confidential documentation Advocate for clients and help remove barriers to care Participate in team meetings and contribute to program development Travel with the mobile unit to various community locations Foster a safe, supportive, and trusting environment for clients What We're Looking For Bachelor's degree required ( Social Work, Psychology, Human Services, or related field preferred ) Experience in case management, social services, or behavioral health preferred Strong communication, organization, and interpersonal skills Ability to work both independently and within a team Comfortable working in a mobile, community-based environment Knowledge of local resources and service networks is a plus Work Environment Mobile unit (bus-based service delivery) traveling to community locations Direct interaction with diverse populations in real-world settings Primarily seated work with occasional standing, walking, and lifting (up to 25 lbs) May involve handling sealed samples with appropriate PPE Why Join Pathway Healthcare?
