Medical Case Management Coordinator Location:
Nashville, TN As a Medical Case Manager at Nashville CARES, you will develop relationships with clients and support them through many stages of life while learning and growing in your own career. The goal of case management is to reduce barriers to accessing healthcare. What You'll Do
  • Provide compassionate and respectful care and comprehensive case management services to help clients address the complex challenges associated with living with HIV
  • Meet regularly with clients in the office, in clients' homes, and in the community
  • Conduct biopsychosocial assessments on all clients on a semiannual basis
  • Assist clients with connections to medical, social, community, financial services
  • Assist clients in developing realistic action steps to achieve their goals
  • Provide support services and referrals to clients who may be socially isolated
  • Provide transportation to clients to access medical, community & agency services
  • Advocate for clients within the agency and in external settings, such as medical providers and other agencies they may be engaged with in the community
  • Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards to ensure clients do not encounter barriers to service
  • Practice appropriate boundaries with clients and with work to ensure you are able to provide excellent services to clients while preserving self-care Who You Are

  • A clear communicator who can explain complex ideas in ways that are easy to understand
  • A problem solver who can see both details and the bigger picture and find balance
  • Can independently research solutions & resources while knowing when to ask for help

  • Someone who is organized and can keep track of a variety of clients and their needs
  • A person with excellent documentation skills
Minimum Requirements:
  • Bachelor's degree (preference for Bachelor's in Social Work or other Human Services)
  • Minimum of two years' experience in community based social or health care services - a supervised BSW/MSW internship may count for one year
  • Proficiency with a variety of technology, including Microsoft Office , electronic health records (training provided), and general PC and cell phone technology
  • Valid Driver's License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work Who We Are

Pay Range:
$50,000-$60,000 - Positions are placed in tiers based on education, experience, and skillset, and this determines the specific pay rate of each employee.
