Position :
Full-time We are seeking a dedicated and compassionate Case Manager to join our team. In this vital role, you will coordinate and oversee individual care plans, ensuring individuals receive the appropriate support and resources to achieve their goals. KEY RESPONSIBILITIES
Assess individual's needs and develop personalized care plans Coordinate services and resources across multiple agencies and providers Monitor individual's progress and adjust care plans as necessary Advocate for individuals to ensure they receive appropriate support Maintain accurate and detailed case records Collaborate with team members and external stakeholders to optimize individual outcomes Provide guidance and support to individual navigating complex systems
SKILLS AND QUALIFICATIONS
Proven experience in case management, social services, or related roles Excellent communication and interpersonal skills Strong organizational and time-management abilities Ability to handle sensitive and confidential information with discretion Knowledge of community resources, healthcare systems, and social services Problem-solving skills and a compassionate approach to indvidual care
Requirements:
MINIMUM REQUIREMENTS
TN Licensed Practical Nurse (LPN) preferred; or Medical Assistant certificates may be accepted as an alternative to LPN with at least one (1) year of experience. or, an Associate's Degree or Five (5) years of healthcare or DDA experience may be accepted as an alternate to a LPN or Medical Assistant certificate. Case management experience preferred. DDA and Managed Care Organization (MCO) designated online training, First Aid, CPR, and Medication Certification course within first 90 days of employment, and any other required training. Person Centered Thinking Training. Emory Valley Center Orientation. Skills System training. Valid TN driver's license and a good driving record. Annual registry, background, and motor vehicle record reviews are part of our commitment to safety.

Establish and maintain a comfortable, supportive relationship with individuals receiving support and their families, ensuring their needs and preferences are prioritized. Develop and implement individualized support plans in collaboration with individuals, families, and service providers. Maintain effective communication and positive relationships with management staff, employees, and the public, demonstrating sensitivity to organizational needs and community perceptions. Stay informed about company operations, including DDA, CHOICES, and Employment and Community First CHOICES programs, to ensure compliance and effective service delivery. Present information, facts, and recommendations clearly and effectively in oral, electronic, and written formats. Exercise good judgment, responsibility, and independence in decision-making to support individuals and uphold organizational Proven ability to build trusting, supportive relationships with individuals and their families. Strong interpersonal and communication skills, both verbal and written. Ability to present information clearly and persuasively in various formats. Demonstrated good judgment, responsibility, and the ability to work independently. Relevant experience in social services, case management, or related fields is preferred.
