General Responsibilities:
The Case Managers are responsible for making home visits to screen persons referred to Meals on Wheels Pus, Inc. for services, as well as making update visits to current clients. Makes special visits to clients as required. Responsible for certain notification and reporting procedures annually. Works under the direct supervision of the Assistant Director.
Specific Responsibilities:
To be responsible for the initial visit within 10 calendar days to persons referred for services within assigned caseload. To be responsible for insuring that services are initiated within 10 calendar days from the effective date of the Texas Health and Human Services (HHS) referral for all HHS clients, as well as Managed Care clients from Amerigroup and Superior. Complete all required paperwork.
This includes the following:
Must complete a Nutritional Risk Assessment annually on all clients age 60 and older, this does not include Managed Care clients. Must complete a form annually that is designed to measure the number of ADLs and IADLs on each client age 60 and older, this does not include Title XX or Managed Care Clients. Provide all clients with information regarding their rights and responsibilities, complaint procedures, privacy of confidential information, and abuse, neglect and exploitation information at least annually. Respect the confidentiality, rights and privacy of all clients. Assist in follow up when clients are not home to receive their meals after two consecutive failed meal delivery attempts. To be responsible for making the determination when a referral is of an emergency nature and making contact with those individuals within 24 hours, or by next work day. To be aware of deadlines as they impact the clients and coordinate activities to insure deadlines are met. Maintain intake log of referrals within assigned caseload. To make referrals to funding and service sources as needed, and follow up on their progress. To be responsible for annual update visits to all active clients within assigned caseload to insure that all forms and notification requirements are completed within the required time frames. Note and report to the appropriate persons or entities any significant changes in the client's physical or mental condition or environment. Document and report any abuse, neglect or exploitation to Adult Protective Services. Inform clients about safety, health or fire hazards identified in the client's home when hazards are discovered. Screen clients for extra services provided by the community, determine specific needs and refer to other agencies/groups when appropriate. Coordinate special projects that benefit clients, such as holiday gifts, home and yard work by community groups, etc. Be responsible for obtaining a written diet prescription from the clients' physician when a low potassium diet is needed. To assist with delivery of meals as needed. This includes training new volunteer deliverers. To assist in answering the telephone and directing calls to proper person as needed. Attend educational workshops and meetings in order to be knowledgeable about other services available for the elderly and disabled in the community, and issues effecting clients. Perform other duties as assigned by the Assistant Director.
