JOB DESCRIPTION
Performs all case work within specified time frames. Maintains full, accurate, and current documentation of evaluations, needs, progress, services, financial data and all other categories of information necessary in each participant's record. Understands and subscribes to the philosophy and values of independent living, empowerment, and community integration. Enables the consumer/participant to make informed decisions and supports these in such areas as housing and employment. Personally knows every participant in the assigned caseload and is familiar with the goals, needs, opportunities, and problems of each. Meets regularly with the participant and his or her allies to discuss progress, problems and plans, making an effort whenever possible to meet where the participant lives, works, attends classes or plays. Maintains a cooperative working relationship with other case managers, the program director, program specialists, and service providers. Performs all such other duties as are outlined in the program manual. Adheres to the strictest standards in respecting participant confidentiality.
REQUIRED EXPERIENCE, KNOWLEDGE, SKILLS OR ABILITIES
Must be able to demonstrate sound independent judgment and work under minimum supervision. Due to the high volume of scheduling requests and participant expectations, the position requires speed, efficiency, and considerable problem-solving skills. The position may require working extended hours including weekends and/or nights. Effective communication skills in both English and Spanish. Experience in direct service delivery. Ability to travel throughout the service area in a personally owned and insured vehicle. Valid Texas driver's license with proof of insurance. Ability to travel outside the service area for training purposes as needed. Proficiency with Microsoft Office (Word, Excel, Access)
MINIMUM REQUIREMENTS
Bachelor's degree in a health and human service field, and/or two years of experience in the delivery of human services to persons with disabilities, or… Be licensed by the Texas State Board of Social Work Examiners at the time of employment or have later one of the following degrees: Master Social Work, Bachelor of Social Work, or Social Work Associate.
