Case Manager - GPD Veterans Reentry (Full-time, Exempt)

Director of Programs and Operations Job Description :
The GPD Case Manager provides housing-focused, Veteran-centered case management services under the VA Grant and Per Diem (GPD) Case Management program. This position is responsible for housing navigation, permanent housing placement, and care coordination for Veterans experiencing homelessness. Services are delivered in alignment with VA Grant and Per Diem Notice of Funding Opportunity (NOFA) requirements and Housing First principles, with the goal of assisting Veterans to rapidly obtain and maintain permanent housing. The Case Manager operates under the clinical oversight of the Clinical Supervisor.
Physical Requirements :
Bending, stooping, standing for extended periods, stretching, stair and ladder climbing and ability to lift 30 pounds.
Principal Duties :
Housing Navigation & Permanent Housing Placement
  • Serve as the primary provider of housing navigation and permanent housing placement services for Veterans enrolled in GPD Case Management.
  • Conduct comprehensive assessments to identify housing needs, preferences, and barriers.
  • Develop and implement individualized, housing-focused service plans in collaboration with Veterans.
  • Assist Veterans with housing searches, rental applications, documentation, and lease readiness.
  • Engage landlords, property managers, and housing providers to facilitate housing placement.
  • Support Veterans through move-in to provide coordination services that promote housing stability.
Case Management & Clinical Support Provide ongoing, recovery-oriented case management using trauma-informed and Housing First approaches. Coordinate care with VA Medical Centers, VA GPD Liaisons, and VA housing programs (e.g., HUD-VASH, SSVF). Connect Veterans to healthcare, behavioral health, benefits, employment, and community resources. Coordination & Collaboration Collaborate with SSVF-funded providers and community partners to support housing placement and stabilization. Participate in case conferencing, coordinated entry, and multidisciplinary team meetings. Maintain regular communication with the VA GPD Liaison to ensure continuity of care and compliance with VA expectations. Documentation & Compliance Maintain accurate and timely documentation in accordance with agency requirements, agency policies, and confidentiality standards. Complete required assessments, progress notes, and outcome reporting within established timeframes. Ensure compliance with VA regulations, GPD performance measures, and quality assurance standards.
Qualifications :
Required:
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related social science field required. Demonstrated ability to provide case management, coordinate community resources, and support individualized service plans. Strong communication, documentation, and organizational skills. Ability to facilitate group activities, coordinate services, and maintain accurate records. Knowledge of housing navigation, permanent housing placement, and Housing First principles. Ability to work effectively with Veterans experiencing complex behavioral health and social needs. High level of organization, attention to detail, and ability to complete tasks with minimal supervision. Basic computer skills including email, word processing, databases and spreadsheets.
Preferred:
Clinical licensure (LMSW, LCSW, LPC, LMFT, or equivalent) preferred but not required Experience working with Veterans and familiarity with VA homeless programs (GPD, HUD-VASH, SSVF). Knowledge of local homeless response systems and coordinated entry. Experience in housing-focused or rapid rehousing service models. Bilingual in Spanish Work Environment Community-based position requires local travel to meet with Veterans, VA facilities, and community partners. Flexible schedule may be required to meet Veteran and program needs.
Additional Requirements :
Employees must successfully complete a criminal background check and motor vehicle records check. A valid Texas Driver's License and reliable transportation are required. Position may require evening and weekend hours. Employees must be available for on-call duties to respond to community emergencies as needed.
