Client Solutions Position Purpose Austin Street Center's Healthy Community Collaborative (HCC) is designed to support adults experiencing homelessness who are living with a serious mental illness or co-occurring substance use disorder. HCC takes a collaborative, community-wide approach to reducing barriers to care by connecting individuals with the mental health treatment, benefits navigation, and resources they need to achieve stability and move toward permanent housing. Participants receive coordinated, wraparound support designed to address the full spectrum of needs that stand between them and stable housing The Case Manager II will carry a specialty caseload of HCC clients, providing consistent, high-touch case management that includes weekly face-to-face contact. This role is responsible for actively connecting clients to mental health resources, benefits assistance, critical document restoration, and utilizing the housing coordination process. The CM II will work closely with behavioral health providers, community partners, and internal program staff to coordinate care and move clients along a clear pathway toward housing stability. Core responsibilities include, but are not limited to: Maintain a specialty caseload of HCC clients with weekly contact. Complete comprehensive housing needs assessments to identify client strengths, barriers, homeless history, and service needs Connect clients to mental health resources, outpatient treatment, and psychiatric services in the community Assist clients in obtaining critical documents including state-issued ID, birth certificates, social security cards, and homeless history documentation Connect clients to referral partners who can assist with accessing SSI/SSDI, Medicaid, and other entitlements Coordinate with behavioral health providers, community partners, and internal ASC staff to ensure integrated, wraparound service delivery Document all client interactions in HMIS Participate in case conferencing and team meetings to support collaborative decision-making Facilitate housing-focused classes as part of ASC's Housing 101 We are looking for a compassionate, organized, and persistent professional who can build genuine relationships with individuals navigating serious mental illness while keeping a clear eye on the path forward. - Bachelor's or Master's degree in social work, psychology, along with experience working in the mental health field.
- Ability to communicate effectively, both orally and in written communication
- Demonstrates experience with and sensitivity to varying cultural, ethnic, and social backgrounds, values, and attitudes
- Intermediate to advanced familiarity with digital communication, including cloud-based software, e-mail, and Microsoft Office applications
- Must be able to lift 25 pounds on a regular basis
- Able to work outside in adverse weather conditions when needed
- Able to work in a noisy environment with a large group of people
