Working alongside the clinical and discharge-planning team, this role serves as a liaison between clients, their identified families or financial supporters, and outside insurance brokers or certified enrollment assisters — helping clients organize their financial and coverage picture so they leave treatment with a clear plan for continuity of care. Essential Duties and Responsibilities
- Meet with assigned clients, primarily in person, to assess anticipated 2027 healthcare coverage needs and complete a financial-planning worksheet (income, household size, current coverage, anticipated needs).
- With a valid, current signed authorization/release of information on file, contact the client's identified family members or financial supporters to discuss the client's coverage plan and available financial assistance.
- Provide clients with general, non-plan-specific education about how the Health Insurance Marketplace, premium tax credits, and cost-sharing reductions work, using CMS/HealthCare.gov consumer materials.
- Help clients gather and organize documentation typically required for a Marketplace application or broker meeting (proof of income, household composition, current coverage).
- Refer each client to a Company-approved, licensed insurance broker or CMS-certified Navigator/Certified Application Counselor (CAC) for plan comparison, plan selection, and application submission; document the referral and confirm hand-off.
- Participate in relevant multidisciplinary treatment team discussions to coordinate coverage planning with each client's broader discharge plan.
- Maintain an active caseload tracker; document every client contact, family contact, and referral in the EMR/designated system within 24 hours.
- Escalate any client presenting acute financial distress, safety risk, or clinical concern to clinical staff immediately, per facility protocol.
- Complete required
