Job Description
Provide and administer Skills Training by conducting individual and family sessions in accordance to the Children & Adolescent Texas Recommended Authorization Guidelines (C/A TRAG) as presented in the Resiliency & Disease Management (R DM) approach. Performs minimally complex social service work and ensures health care services as needed to clients. Facilitates and arranges for the communication of information between clients to psychiatric and community services. Work under moderate supervision with latitude for the use of initiative and independent judgment. Provide intensive case management by providing school and/or home visits; provide on-call coverage for TCOOMMI clients. Provide excellent customer service and must be able to work with probation officers, courts and community agencies.
Assesses client's needs and authorizes services to meet the identified needs.
Coordinate in the development of a Person Centered Plan with measurable outcomes.
Coordinates service delivery.
Provide Crisis Prevention and Case Management services when needed. Monitors service delivery.
