Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Maintains a working knowledge of community resources; Coordinates referrals to community agencies and tracks outcomes of referrals; Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients; Documents all client encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client; Reviews encounters and records of community prevention workers; Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community; Coordinates organizational and community groups that are working to address social determinants of health; Develop and implement educational programs; Assists with the development of policy and procedures; May supervise or coordinate the work of community workers; Plan goals and objectives for assigned programs; Assist in budget preparation; Assist with health department social media; Additional duties as assigned. Experience in case management or social services work. Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred.

Valid Texas Driver's License Required Knowledge and Abilities:
