Position range in Lubbock County $27k - $46k Per Year CMHG Jail in Reach Program Case Manager
Work is performed under minimal supervision of the Forensic Diversion Programs Manager, Director of Forensics, and Director of Diversion Services, with extensive latitude for the use of initiative and independent judgment. TheCMHG Case Manageruses a collaborative approach to align and coordinate services within Hockley County Jail for incarcerated individuals with mental health and substance abuse issues and needs. This role assumes responsibility for assigned projects and provides case management services to those served in the CMHG program through mental health services, reentry, and jail-based competency restoration services when applicable. Services include case management services for those served in Jail Based competency restoration incarcerated at Hockley County Jail, support services for those incarcerated with a mental health diagnosis or cooccurring diagnosis, as well as assisting with intake screenings for those booked into Hockley County Jail for the purpose of identifying people with intellectual and development disabilities, mental illness, and/or substance addiction.

Responsibilities:
Provide case management to identified inmates at the Hockley County Jail. Work effectively with CMHG staff, inmates, and outside support agencies as necessary. Maintain the confidentiality of consumer information in accordance with StarCare and Hockley County Jail policies. Completion of specific duties and responsibilities as designated by the director. Designation of duties will be determined by current needs of the consumer population. Provision of crisis intervention services to consumers as referred and deemed appropriate for the caseload. Crisis intervention may require consultation with CMHG and MCOT staff regarding management/treatment concerns, referral, and/or mental health follow-up. Completion of Case management Service Reports/notes of consumers assigned in a timely manner. Completion of regular reports of the employee's activities in accordance requests of the Forensic Diversion Programs Manager. Provision of support and/or monitoring of patient's mental condition for assignment to a less intensive caseload. Participation in staff meetings, CMHG meetings, and training as necessary. Participation in specialized case management services and/or program development activities in a professional, timely manner providing these duties have been assumed with the mutual agreement of the Forensic Diversion Programs Manager.Schedule:
This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.Benefit Package:

Graduation from an accredited four (4) year college or university with major course work in a Human Services Field, (Sociology, Psychology, Family Studies, etc.),AND two (2) years of full-time experience in a related field,required. Bilingual (English/Spanish) preferred.Skills & Abilities:
Knowledge and skill in operating office equipment (phone, fax, copier, etc.) and personal computer with Microsoft Office products is required. Excellent communication and interpersonal skills. Ability to work collaboratively with diverse teams. Strong problem-solving and analytical skills. Ability to manage multiple projects simultaneously and meet deadlinesAdditional Requirements:
Must pass a pre-hire drug screen and criminal background check. Must have a Texas driver's license, personal auto liability insurance, and be insurable under the Agency's insurance. Local travel, with mileage reimbursement, is required.Similar remote jobs
