Health Home Case Manager Truemed Home Care - 4.5 Mission, TX Job Details Full-time From $13 an hour 11 hours ago Qualifications Teamwork Customer service Attention to detail Organizational skills Delegation Full Job Description Job Overview We are seeking a proactive and detail-oriented Case Manager to join our team. In this vital role, you will support a wide range of Home Health Provider Agency functions. Your energetic approach and meticulous attention to detail will help foster a positive work environment, streamline Case Manager processes, and enhance employee engagement. Duties Manage data entry , ensuring accuracy and confidentiality Design, execute, and adjust the Plan of Care (POC) alongside physicians, therapists, and the patient's family. Maintain accurate clinical notes and adhere to Medicare or private insurance reimbursement criteria. Supervise and delegate duties to Home Health Personal Attendants. Assist with talent acquisition activities, scheduling interviews, and supporting onboarding processes Support employee orientation and facilitate new hire process Coordinate employee evaluation and performance management processes in collaboration with managers. Maintain clients records in compliance with state requirements Proven experience in customer service Excellent communication skills to effectively interact with employees at all levels Ability to handle sensitive information with discretion and maintain confidentiality Demonstrated organizational skills with the ability to manage multiple priorities efficiently
