Job Description

Job Overview Provides direct case management services for pregnant and parenting mothers experiencing homelessness who are residing at Mater Maria maternity home. Duties include intake, service plan writing, safety plans, referrals, completing government program applications, connection to post-graduation programs such as food pantries, support groups, arranging medical services; individual counseling, assistance/guidance in decision making, and planning for the care of the child.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides direct case management services to pregnant and parenting mothers and their babies. Provides referral services and information for effective access to community resources such as WIC, SNAP, and health insurance Provides relevant education/training to pregnant and parenting mothers Ensure residents participate in program engagement and house responsibilities Provides mentoring services for pregnant and parenting mothers Assist with maintaining and updating the maternity home's schedule, ensuring all classes, activities, and appointments are organized and that residents are aware of their commitments. Help with planning and organizing community-based activities or events for residents to participate in. Help residents obtain Social Security cards, birth certificates, IDs, and driver's licenses. Completes contract billing requirements Maintains minimum standards and file audits Capture and report client success stories to better enhance program advancement. Supervises interns and volunteers
OTHER DUTIES
Evaluates cases and makes determination for closure when appropriate Completes documentation in an accurate and efficient manner Outreach services to increase number of residents at Mater Maria Workshops and trainings at clinics, hospitals, agencies, etc. to increase awareness about pregnant women experiencing homelessness Adheres to applicable professional and agency code of ethics. If applicable, maintains licensure and certification: attend mandatory trainings, in-services, continuing education, and staff meetings. Supports the Missions, Values, and Vision of Catholic Charities of Dallas. Other duties as assigned by residential supervisor. Adhere to policies related to boundaries with and protection of clients. Complete required abuse prevention training (Safe Environment) and others, as required by program. Report suspicious and inappropriate behaviors. Follow mandated abuse reporting requirements. Adhere to job specific abuse risk management responsibilities. Adhere to procedures related to managing high risk activities and supervising clients, as applicable.
EDUCATION & TRAINING
Bachelor's degree in related field; 2 years of casework experience preferred. BSW; LMSW or LCSW preferred.
KNOWLEDGE, SKILLS & ABILITIES
Maintains current professional license for the state of Texas, if applicable. Maintains professional liability insurance, if applicable. Computer literate and experienced with MS Office suite of products. CPR/First Aid Trainings as required by program. Excellent interpersonal and communication skills; verbal and written communication skills. Strong organizational and time management skills required.

Valid TX driver's license and required insurance
Health:
New Hire TB screen. Other public health screenings or immunizations as indicated by program.
Language requirements:
Fluent in English and Spanish (preferred)
Travel:
25% of travel required
Pay:
$21.00 - $23.00 per hour
