The Collaborative Care Case Manager (CCCM) will provide services for up to 15 individuals in Bluebonnet Trails Community Services Outpatient Biopsychosocial Services for Persons with IDD Program (OBI). The program is designed to assess, offer treatment services, and make referrals to other needed services and support in a biopsychosocial framework (or providing services to the whole person). The duties of the CCCM include the provision of case management and collaboration with mental health providers to support individuals with IDD, as well as their family/support system, to understand their co-occurring mental health and/or substance use diagnoses and how to increase their involvement in their recovery, as well as to provide support during a crisis. Duties will include addressing barriers that impede access to services and providing resources to ensure the appropriate delivery of those services. Referrals for needed mental and physical health services will be provided to the individual/support system through the CCCM; and follow up on the results of these referrals will assist individuals to access additional services to support their recovery. The CCCM will facilitate the development of a person centered, strength-based treatment plan with each individual and their support system, which will serve as a guide for goals and skills training. Timely follow up with the individual and/or their support system is expected when they are hospitalized in psychiatric facilities or incarcerated, to begin timely and effective discharge planning or diversion, where possible. The individuals served will be of all ages and will be residents of Williamson or Burnet Counties. Services can be provided tele-video, face to face, and telephone as appropriate to the needs of the individual/support system. The CCCM works with families and demonstrates sensitivity to various cultural differences and demonstrates ability to establish relationships within the agency to facilitate needed supports, as well as with external partners. The CCCM will participate in data gathering elements of the program to ensure timely reporting is accomplished and as directed by supervisor.

  • Associate License or Advanced Licensure .50/our
LCDC Minimum Qualifications:
Bachelors degree from an accredited college or university with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education or criminal justice. Must have at least one year experience providing services to individuals with intellectual disabilities. Must have valid Texas drivers license and maintain approved drivers status throughout employment.
Preferred Qualifications:
Two-three years experience providing services to individuals with intellectual disabilities. Experience providing mental health services. Experience providing crisis services. Bilingual (Spanish/English or ASL/English). Experience providing services to persons with substance use disorders. Experience providing case management or service coordination.
