Position Summary:
This position is responsible for the provision of Service Coordination activities and assessments to assigned caseload. This includes, but not limited to, maintaining all required monthly contacts, linking resources of various providers to consumers, advocacy, and participating in the judicial system in accordance with the philosophy, goals and objectives of Texoma Community Center. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.
Qualifications:
Education and/or
Experience:
Bachelor's Degree in Social Work, Sociology, Psychology, Criminal Justice, Education or Counseling as outlined in TAC Human Services Experience preferred Ability to speak a second language, relevant to the population served, is preferred 1 year of experience working with individuals with IDD preferred
Certificates, Licensure, Registrations:
Current driver's license and vehicle for use at work
Knowledge, Skills and Abilities:
Excellent communication skills (verbal & written) and the ability to interact with diverse populations are essential Knowledge of Office Suite Consistently practices Trauma-informed Model of Care, understands and considers the pervasive nature of trauma. The knowledge of cultural differences and ability to work with individuals from different cultural backgrounds. Ability to speak a second language relevant to population served preferred
