Position Summary:
Initiates preliminary TRRs and recovery plans for all consumers seen in triage within 7 days, in accordance with CCBHC guidelines. Establishes new consumers coming into services and facilitates transition into the case management team. Provides rehabilitation case management or service coordination services as outlined in the Texas Administrative Code and current HHSC guidelines for persons with persistent and severe mental illness including advocacy, crisis services, individual and group skills training in accordance with the philosophy, goals and objectives of Texoma Community Center. Other duties as assigned. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.Qualifications:
Education and/orExperience:
Bachelors Degree in Social Work, Sociology, Psychology or Counseling with two (2) years Human Services experience requiredCertificates, Licensure, Registrations:
Current drivers license and a vehicle for use at workKnowledge, Skills and Abilities:
Ability to speak a second language relevant to population served preferredMust be self-motivated, organized and detail orientedKnowledge of Microsoft Office Suite Consistently practices Trauma-informed Model of Care, understands and considers the pervasive nature of trauma.The knowledge of cultural differences and ability to work with individuals from different cultural backgrounds.
