PURPOSE OF POSITION
Case Manager under the direction of the Executive Director, is responsible for working during the day, nights, weekends, and holidays if needed. Case Manager is an energetic, creative, and diligent member of a team that includes resident advisors, and a cadre of specialty staff including but not limited to mental health staff. The Case Manager will provide comprehensive case management services to the client and children who are on their caseload including: creation of individual service plans, coordination of services, individual counseling, advocacy, monitoring of the client's efforts and challenges in completing goals, monitoring the safety and development of the children, and celebrating goal achievement. The Case Manager will contribute to a program culture that is person-centered, holistic, focused on positive client development, and committed to continuous improvement. The Case Manager is expected to participate in client development and other therapeutic, parenting skill, and life skill activities for the benefit of the client and their children living at the site, as well as staff meetings including case review meetings to assess clients' progress. Ultimately, Case Managers assist in a holistic way the achievement of goals of independent living: stabilize and address any immediate trauma experienced by the client in our care, maintain housing stability, and assist the client in advancing towards their individualized goals including a long-term housing plan, while ensuring the site is well-maintained. Additionally, this program will utilize evidence-based and other proven models; the Case Manager will be expected to participate in all training and implementation activities as needed. Case Manager will need to be comfortable in a rapidly changing environment where processes and protocols are being developed and refined as the program is operating. Additionally, this position requires independent judgment and proactivity, flexibility, a willingness to be available by email or phone, and excellent communication and problem-solving abilities are critical to successfully fulfilling the requirements of this position.
Monday- Friday Shift:
8:00 am - 5:00 pm
DUTIES AND RESPONSIBILITIES
(Responsibilities to include but not limited to the following) Case Management Provides integrated counseling and case management services that effectively engage mothers and promote personal growth and wellness, the safety and healthy development of clients' children, housing stability and accomplishment of service plan goals on an ongoing basis. Collaborates with pregnant women to assess strengths and challenges and develop an individual service plan that reflects the client's progress and stated goals. Meets with client and their children regularly to determine progress with goals identified in the ISP. Advocates on behalf of client and their children with outside service providers and other collateral services. Assists mothers in the development of nurturing and safe parenting skills. Creates and maintains confidential records, charts and logs for youth documenting all relevant information from individual and group interventions as well as with collateral services. Ensures that clients' files are compliant with all regulatory and other requirements. Completes incident reports per agency policies. De-escalates crises; attends to emergencies following agency protocol, as needed Perform other duties as assigned and needed. Team Participation Works as a collaborative member of the team ensuring all clients have access to high quality services and are living in a safe and compassionate community. Participates in and facilitates regular case conferences. Ensures team members are fully apprised of important information regarding client as needed. Participates in staff team meetings and trainings, individual supervision meetings, and meetings with client. Assists with audit preparation and dry and live audits of the program. Demonstrated knowledge of and experience working directly with homeless pregnant women through approaches that emphasize resiliency, strength-based client development, and trauma-informed care required.
KNOWLEDGE, SKILLS AND ABILITIES
Education and Experience Must be at a minimum of 25 years of age. Bachelor's degree required, preferably in a relevant field.
Case management:
2 years (Required One (1) year of experience in a residential setting working with a homeless population preferred. Experience working with a homeless population strongly preferred. Working with individuals with mental health issues: 1 year (Preferred) Mental health/and or trauma setting: 1 year (Preferred) Driver's license preferred. Comfortability driving a 7-passenger van a plus. Must have no disqualifying criminal history; must be willing to submit to a thorough criminal background check. Must be free of tuberculosis (TB) and submit to a TB test. Must submit to and pass a drug test. Knowledge/Skills Strong multi-tasking skills with demonstrated ability to manage multiple projects at once required. Excellent verbal, written and problem-solving abilities and an upbeat demeanor required. A strong focus on child protection and ensuring the safety of residents required. Experience working with diverse groups required. Overall flexibility in work schedule required, including weekends, evenings and holidays if needed. Experience utilizing evidence-based and other social service models preferred. Dedicated to the mission of Our Lady of the Angels Maternity Shelter.
Physical Requirements:
Bending, lifting to 25 pounds, grasping, fine hand coordination, pushing and pulling. Ability to supervise clients and their children by sight and sound and be aware of the activity, location and who they are with. Ability to administer CPR.
